Terms & Conditions

You should only attend an appointment once you have read and agree to the following terms and conditions. 

In the event you are unable to attend an appointment you must give 24 hours notice of cancellation to receive a refund or transfer your appointment to another date. 

Following booking an appointment you will receive an email requesting you to complete a form detailing relevant medical information. To attend an appoinment you must fill out this form and give consent to either in-person or online program advice and participation. You can download a copy of the form you will receive below. 

​ At online appointments consideration for practicing in a safe environment, free of trip hazards should be considered and you should only use equipment appropriate to the session.
